Committee Rules: The committee has been formed under Article XI of the MHMA Bylaws. The MHMA board appoints committee members for one year. The committee chair will notify you if your application has been accepted and will invite you to join the committee. Please use the comment section to add as much detail as possible to help the board understand why you are suitable for a particular committee.

By checking the box below, I acknowledge and agree with the MHMA board's right to APPROVE or DECLINE my application to join the  Committee. If APPROVED, I promise to serve as a member of the committee to the best of my ability in compliance with the MHMA bylaws and within the guidelines provided by the MHMA board.
